One record per entity, every revision kept.
teleon.dev/docs/entities-db holds canonical entity records and their annotation revisions. A record keeps its identity for life; every change to its description is a new, dated revision.

Each change is a new revision with two dates; the record's ID stays the same.
What it does
Canonical IDs
Each entity resolves to one ID, whatever name it arrives under.
Revisions kept
Annotations are immutable revisions, so history reads back exactly.
Bitemporal facts
Attributes record when they were true and when they were learned.
The contract
Entities are stable; annotations append; links between entities live in the graph cell.

How it connects
For agents
Give your agent the read-only guide first. It reports back before any write.
